Mental Health Consumers' Perspectives of the Implementation of Recovery Principles

Purpose of the Study
The purpose of this study is to determine recovery experiences of mental health consumers receiving services through Vancouver Community Mental Health Services (VCMHS). You are being invited to take part in this research study if you are an adult who has received services from the VCMHS sometime in the past three years. 
Study Procedures 
This study consists of completing a questionnaire: Recovery Self-Assessment, and answering some general demographic questions, this should take about ten minutes to complete. The questionnaire can be accessed online, see the link below.

When completing the questionnaire please note that "your agency" refers to the Vancouver Community Mental Health Services.

Potential Risks
There are no risks believed to be associated with this project as participation is voluntary and no individuals will be identified. No identifying informaiton is to be put on the form. Your confidentiality will be protected.
Potential Benefits
It is hoped that the study will be informative, positive and educational for the participants. The results of this research are intended to help the VCMHS deliver more recovery-oriented services. 

Confidentiality
No individuals are identified in this study and no identifying personal informaiton will be collected. Documents will be identified by a code number and will be securely kept for five years and then destroyed.  

This online survey company is hosted by a websurvey company located in the USA and as such is subject to US laws, in particular, the US Patriot Act which allows authorities access to the records of internet service providers. This survey or questionnaire does not ask for personal identifiers or any information that may be used to identify you. The websurvey company servers record incoming IP addresses of the computer that you used to access the survey but no connection is made between your data and your computer's IP address. If you choose to participate in the survey, you understand that your responses to the survey questions will be processed in the USA.
